Professional Geriatric
Care Managers
Perhaps you have an elderly parent who has suffered recent falls, or who seems increasingly confused? Maybe you have found a caregiver for your elderly friend, but wonder if this caregiver has the proper training or supervision? Or maybe you are struggling with your own long-term care insurance? The issues that often arise as someone ages can be confusing and daunting… and unfortunately, the options and solutions can be just as complicated and overwhelming.
As geriatric care managers, our job is to identify and evaluate these issues in your situation, help you understand your choices and solutions, and provide a plan that maximizes care and quality of life while being sensitive to individual concerns, family dynamics and budget issues. We are full-service care managers, which means that we do everything from one-time consultations or assessments to on-going care management, where we can provide and/or coordinate any service that an elderly person might need, including bill-paying, housekeeping, caregiver training, placement in an assisted-living or skilled nursing facility, and transportation to appointments.
Please take this opportunity to learn more about the Conners Care Management Team and our 30+ years of experience, geriatric care management in general (including examples of geriatric care management in action), and the many ways we can help you or someone you care about. We are always happy to speak with you about your specific situation in person, over the telephone, or by email. There is no charge until you decide to use our services.
Warmly,
Meg Conners R.N., Julie Conners
and the Conners Care Management Team
