“Care management” is a relatively new concept in health care, but the underlying goals are well-established:
We seek to efficiently provide and coordinate medical, personal and social service resources
to maximize the quality of life for older adults and their families.
Our services include:
*Assessments: including cognitive, competency, living environment, social and other needs
*Development of comprehensive care plans
*Household services such as bookkeeping, housekeeping, gardening, shopping and transportation
*Referrals to service providers: physicians, home health care, assisted living and skilled nursing facilities, etc.
*Referrals to other professionals: attorneys, accountants, conservators, wealth managers, social workers, etc.
*Implementation and coordination of ALL services, including referrals
*Consulting and education on a wide range of geriatric issues
*On-going monitoring of care and services to ensure continuing quality of life
*Caregiver training and/or monitoring
*Consultations available in person, or by telephone, email or fax
*Communication with family members, care providers and/or other concerned individuals
*Assistance with insurance issues
How we function:
We can operate as consultants, providing one-time assessments with little or no follow up.
We can also serve as full-service care managers, doing everything from initial assessment, recommendations, care plan creation, and implementation and on-going management of the care plan.
