Here are several examples of how Conners Care Management can make a difference:

*A concerned son contacted us about his elderly mother, who was blind and living alone. She had suffered several accidents at home and seemed to have declining personal care and weight loss. We did an assessment and then developed and executed a comprehensive care plan, which included placement of a home health aide from a local agency and installation of safety measures. The aide and the safety measures allowed the mother to continue living independently in her own home, with greater safety and comfort.

*A conservator brought us in when he realized his client was not properly managing her medications. We assessed the situation, and implemented several measures including finding a more attentive physician, installing a better system for medication management, and training the client in the use of medical equipment at home. We visit the client once a month to ensure her continued success.

*An out-of-state family was worried about an increasingly confused relative. We found that the man, living alone, had been wandering at night, had advanced dementia, and was suffering from poor nutrition. After a cognitive assessment, we helped his family place him in an Alzheimer’s facility closer to them.

*A daughter from the Bay Area, concerned about her father’s recent falls, asked us for an evaluation of her parents’ living situation. Navigating the family dynamics, we were able to guide her father to a local assisted living facility and to provide household help for her mother, who chose to remain at home. We even coordinated transportation for the mother to visit the father on a regular basis. We now provide weekly monitoring, as a comfort to the out-of-town daughter.
